Background of the Study
Community-based nursing interventions play a crucial role in promoting health and preventing disease within local communities, particularly in regions with limited access to specialized healthcare services (Ayodele & Kalu, 2023). Borno State, located in northeastern Nigeria, has faced significant challenges, including insecurity, poor healthcare infrastructure, and limited access to essential medical services, especially in rural and remote areas. In this context, community-based nursing interventions can bridge the gap between healthcare facilities and underserved populations, providing essential services like immunization, maternal care, health education, and chronic disease management.
Nurses working in the community setting have the opportunity to directly impact public health by addressing common health issues at the grassroots level. However, the effectiveness of these interventions in improving health outcomes in Borno State is underexplored, particularly in the face of ongoing conflicts and resource limitations. This study aims to assess the effectiveness of community-based nursing interventions in Borno State, focusing on their impact on health outcomes, accessibility, and overall community health.
Statement of the Problem
Borno State faces substantial health challenges, and while community-based nursing interventions are an essential aspect of public health delivery, their effectiveness has not been sufficiently documented. The combination of a fragile healthcare system, insecurity, and socio-economic constraints makes it difficult to assess the true impact of these interventions. Nurses, often operating in resource-poor settings, may encounter difficulties such as limited training, scarce resources, and logistical challenges in delivering care. This study seeks to evaluate the role of community-based nursing interventions in improving health outcomes in Borno State, particularly regarding disease prevention, maternal and child health, and the management of chronic diseases.
Objectives of the Study
1. To evaluate the effectiveness of community-based nursing interventions in improving health outcomes in Borno State.
2. To assess the barriers and challenges faced by nurses in implementing these interventions in the community setting.
3. To recommend strategies to improve the delivery and impact of community-based nursing interventions in Borno State.

3. Providing additional training and resources for community-based nurses in Borno State will improve the effectiveness of health interventions.
